Fund updates from previous years are available to view at https://disclose-register.companiesoffice.govt.nz/. Click ‘Search for an offer’. Then, enter the name “Christian KiwiSaver Scheme” in the Offer tab, then click on the “Investment Options” tab.

Withdrawal forms

Retirement withdrawal

This form is to be used when you have reached your qualifying date. This is when you have been a member of KiwiSaver for at least five years and you have also reached New Zealand Superannuation age – currently 65.

Once completed the form provides us with instructions for payment of your withdrawal amount. It also includes a statutory declaration. This is to confirm your eligibility for the Government Contributions you have received (by declaring your residency in New Zealand during the relevant period of your KiwiSaver membership.)

If you have not already done so, you must verify your identity and residential address before you can access your savings.

First home purchase

Use this form if you would like to make a withdrawal to be used towards purchasing your first home or (if you have previously owned a home) a withdrawal approved by Housing New Zealand.

You need to fill in and return this form to us with a copy of your Sale and Purchase Agreement at least a week before you need the money. The form includes a statutory declaration. Your lawyer also needs to fill in and return their section of the form (the Solicitor’s or Practitioner’s Letter).

If your Sale and Purchase Agreement is conditional then you can choose whether the amount withdrawn will be applied towards paying a deposit or applied at settlement. If the agreement is unconditional, the amount withdrawn can only be applied at settlement.

Financial hardship

If you are a member of our Scheme and wish to apply for a significant financial hardship withdrawal, please contact us by emailing us at

Serious illness

To apply for a withdrawal due to serious illness complete and return this form.

Serious illness is an injury, illness or disability that:

  • results in your being totally and permanently unable to engage in work for which you are suited by reason of experience, education or training (or any combination of those things); or
  • poses a serious and imminent risk of death.

You need to fill in and return this form to us. The form includes a statutory declaration. Your doctor also needs to fill in and return their section of the form (the Doctor’s Form) along with supporting documents.

You will be advised of the Trustee’s decision.

If you have not already done so, you must verify your identity and residential address before you can access your savings.

Permanent emigration (excluding Australia)

This form is to be used if you permanently emigrate from New Zealand to another country except for Australia. There is a different form for transferring to Australia (please contact us).

You may apply for a withdrawal after a period of one year from the date of permanent emigration from New Zealand.

You need to fill in and return the form with documentary evidence of your overseas residential address (e.g. utility bills, bank statements) and the date you left New Zealand (e.g. airline tickets, passport stamp).

If you have not already done so, you must verify your identity and residential address before you can access your savings.

Deceased member

Contact us if you want to make a withdrawal on behalf of a deceased member’s estate.

Member information

This form is to be used if your information has not been previously provided to us. This normally occurs when you are automatically enrolled through your employer.

It is to help us help you make the most of your membership of the Christian KiwiSaver Scheme. If you joined the Christian KiwiSaver Scheme automatically through your employer (by way of auto-enrolment or an opt-in) you may not have completed an application form. Consequently, we may not have the details that we need in order to manage your investment in the most efficient way. The form asks for personal details (name, contact address etc), your tax rate and investment choice, and identity verification.

Change of investment choice

Use this form if you are an existing Christian KiwiSaver Scheme member and wish to change your investment strategy. If you are unsure about choosing an investment strategy, you may wish to seek advice from a financial adviser.

Automatic payment

If you wish to make an automatic payment please contact us on 04 473 9369 or 0508 738 473 for bank account details. We are here to help you.

Direct debit authority

Complete and return this form to us if you want us to debit your account regularly for contributions. We can debit your account fortnightly or monthly. Please indicate which payment frequency you would like on your form.

Employer chosen application

If your organisation’s primary activities are in our opinion Christian mission and/or ministry, you can select Christian KiwiSaver Scheme as your employer chosen KiwiSaver scheme.

Identity verification

This guide will help you verify your identify.

Before we can pay any savings to you, we are required to obtain from you proof of your identity and proof of your residential address. This is required under the Anti-Money Laundering and Countering Financing of Terrorism Act 2009. The guide sets out the reason for this request and the types of documentation acceptable, and who can certify your documents.
